| | | Hillsborough Updates
Schedule Changes for Hillsborough: As has been previously announced with the release of the schedule, there will be a period next quarter (5/29 – 6/17) where we will NOT have the Bluebird attending service. We are discussing changes to team caps to accommodate the change – more info to come.


| | | | | | | | | | | | Communication Best Practices: To continue the work of the “secure chat” project, we have been in discussion with nursing about best practices for provider and nursing communication. Our goal is to continue to improve our collaboration with nursing staff at Hillsborough. MORNING TOUCH BASE: The nurses appreciate daily updates about the plan of care for the day. This will allow them to help communicate expectations to the patient.

  • When rounding, try to find the nurse to discuss the plan of care for the day.
  • If the nurse is unavailable, please send a secure chat to provide a quick update.

CLEAR COMMUNICATION AND PLANNING: Collaborate with nursing during the day for anticipated complex needs (ie pain management) to ensure consistency both for day shift and overnight. DISCHARGE EXPECTATIONS: Please notify the bedside nurse and charge nurse of the unit of your anticipated discharges for the day. This can happen via secure chat.
Discharge Hospitality Center (DHC) at Hillsborough Just a friendly reminder: We now have a discharge hospitality center for patients awaiting transportation at Hillsborough. Please work with nursing to identify patients who can safely wait in DHC on day of discharge.

| | | MDU UpdateDischarge Planning Considerations

  • Please AVOID sending individual chat messages to CM/SW from different team members with similar or the same questions. Just add all pertinent team members to the Epic chat so:
    • Everyone has the same information which reduces confusion.
    • Reduces total number of chat messages for CM/SW.

  • Please encourage residents to send one secure chat message to the CM and Eric Allman in the early morning with potential discharges for the day. The CM will relay that information to other members of their team and the charge nurse for the day.
  • As part of morning rounds presentations, encourage the team (especially interns) to present when the patient will be medically ready and EDD.

    • Interns are entering the final last 3-4 months so should be actively involved in discharge planning for their patients and would encourage them to start coming to IDR for their patient.
    • Remember medically ready may be different than EDD (if placement delays). MD/APP should document when medically ready in Epic but ONLY use if medically ready (like you can place the discharge order now not tomorrow).
  • Please encourage utilization of the DHC.

  • Remember to utilize the throughput specialists when appropriate.
  • Remember if your patient came from an outside hospital and no longer needs quaternary or specialty care at UNC, please utilize back transfer. You do need patient permission to start the process and it helps to think about how you present it to patients before talking to them. CCC-W (formerly PLC) should now facilitate ALL outbound transfers to other acute hospitals.

Teaching Topics As a part of my newsletter, I will be covering some teaching topics including: setting expectations, giving feedback, evaluations, techniques for teaching on the wards. If you have a specific topic that you would like addressed, please contact me.

  • As residents and interns approach the last quarter of the year, encourage more involvement in teaching. Third year medical students are starting the first clinical rotations now so they may need help to make a successful transition.
  • Be intentional. Ask the resident about which patients that they plan to do teaching on during rounds and what topics. It encourages them to think about it ahead of time and be more efficient with teaching on rounds.
  • Remember physical exam teaching on rounds or in the afternoon. It is extremely useful for the new medical students starting in March. Many of them have never seen patients with liver failure, heart failure etc. Another quick exam topic for students: different types of IV access and ways to deliver respiratory support.

| | DEA Delay Information: The DEA is currently experiencing significant delays in processing licensure renewals. Processing may take up to 10 weeks, which could result in renewals not being completed before the expiration date. This may affect your ability to prescribe and could lead to NC Medicaid enrollment suspension or termination. To avoid interruptions, we strongly recommend that providers review their DEA expiration date and submit renewal applications at least 60 days prior to expiration.Important Reminder:Do NOT pay for your DEA renewal. We are tax exempt and do not pay a fee for DEA renewals. If you have questions or need assistance, please reach out to the DHM Admin Team.
