Deterioration Index (DI) Score

The DI Score is a computerized clinical acuity score used for prediction of decompensation events. When monitored and communicated effectively, it can save lives.

STEP 2 When a patient's score increases by 10 or more during a shift, clinically evaluate patient if indicated.

Decide if you need to see patient at bedside

  • For a timeline view, look at Deterioration Index Accordion Report on the summary tab
  • For non-English speaking patients, call an interpreter (*468 or 1-855-456-5224) and activate rapid response or critical response nurse consult.

STEP 3 Use clinical judgment to determine next steps.

Assess frequency of patient monitoring

OR

Activate Rapid Response (RR) if patient meets criteria .

If patient doesn't meet RR criteria per guidelines, place a Critical Response Nurse Consult Order.

STEP 1 Wrench the 'Deterioration Index Score Info' into your patient list. Mark scores as 'reviewed'.

'Change' indicates how much the score has gone up or down since last reviewed. Double-click the binoculars to renew the time stamp.
