UNC Hospital Medicine Moonlighter Roles/Responsibilities

This is meant to be a brief guide to how schedule designations match with daily roles and responsibilities. Please review specific Service Guides for details on individual roles and Admissions and Discharges in Processes for expectations when work in admitting roles. CH 2nd (9 AM to 7 PM daily): - Perform new admissions and transfers to hospitalist teams. - Perform new consults for MDM if needed. - On Sat/Sun, contact inpatient (rounding) attendings on arrival to see if assistance is needed - On weekdays, take sign out from inpatient attendings around 4PM and sign into pagers (cross cover between 4PM and 7PM); on weekends take sign out from inpatient attendings around 3 PM. Cross cover is your primary responsibility between 4PM and 7PM, although there may still be opportunity/need to help the Swing admitter with admissions. Sign out any significant cross cover to the Night2 provider at 7PM. - Sharepoint documents to review: “2nd Shift + Swing Shift”, and the 3 Comanagement documents. May also benefit from reviewing “Admissions and Discharge” process review. - For the cross cover portion of the shift, it may be helpful to review the “Rapid Responses and Codes” process as well as the “Significant Event Note” section.

CH Swing (2 PM to 12 AM daily): - Perform new admissions/transfers to the hospitalist teams. - Perform new consultations for MDM if needed. - Please sign in to the Medicine Consult Attending pager at 4PM. - Sharepoint documents to review: “2nd Shift + Swing Shift”, and the 3 Comanagement documents. May also benefit from reviewing “Admissions and Discharge” process review. CH Night 1 (9 PM – 7 AM): - Perform new admissions/transfer to the hospitalist teams during entire shift - Assist Night 2 APP with cross-coverage issues/Rapid Responses as needed - Staff urgent med consults with MDM resident if needed. - Please sign in to the Medicine Consult Attending pager at 9PM. - Sharepoint documents to review: “Night 1”, and the 3 Comanagement documents. May also benefit from reviewing “Admissions and Discharge” process review. CH Night 2 (7 PM – 7 AM): - Provide cross-cover for all MDH services. - Sharepoint documents to review: “Night 2”. May also benefit from reviewing “Rapid Response and Codes” as well as the “Significant Event” process reviews.

HB 2nd (11 AM – 9 PM): - Cover new admissions/transfers to HBC - Serve as primary back up for HBD peri-operative APP and HBR CARES APP when needed - Provide cross-cover on HBC/HBD/CARES after providers sign out - Run code team when ICU provider is not present - Cover MAP transfer center pager from 7 pm – 9 pm - On the weekends or when the APP is not present, round on HBD (Consult) and CARES (long term care) patients who have been signed out as “needs to be seen”\ - Sharepoint documents to review: “HBR Handbook”. May also benefit from reviewing “Admissions and Discharge” as well as “Transfer Center Guidance” process review. HB Night (9 PM – 7 AM): - Cover new admissions/transfers during entire shift - Provide cross-cover on HBC/HBD/CARES - Run code team - Cover MAP transfer center pager - Sharepoint documents to review: “HBR Handbook”. May also benefit from reviewing “Admissions and Discharge” as well as “Transfer Center Guidance” process review.


For Fellows with attending privileges only. All daytime rounding roles start at 7 am and finish when work is complete. Sign out to evening providers should not occur before 4PM on weekdays or 3 PM on weekends/holidays. Individual service lines have their own summary documents posted in SharePoint. CH MDU: - Attending for the Med U teaching service CH MDL: - Attending for the Med L teaching service CH Dogwood, Magnolia, Pine, Cedar: - Attending for hospitalist direct care services. See corresponding service guides in SharePoint. - Provide direct care to assigned team patients - Serve as primary back up for assigned APP for assistance/RRTs CH Consult: - Attending for the MDM consult resident for general medicine consults - Cover admissions between 7 AM and 9 AM. CH Pediatrics: - Pediatrics responsibilities as assigned by that department

HB HBC: - Attending for the HBC direct care service - Provide direct care to assigned team patients - Admit new patients from 7 am – 11 am - Provide back up for HBD/CARES APPs from 7 am – 11 am if requested
