How to Request a Travel & Expense (T&E) Card¶
The T&E card is used for annual memberships, subscriptions, and travel for conferences . Expenses will be charged to your CME funds.
Please note:¶
- Licenses and ABIM fees should be paid with your personal credit card and then submitted for reimbursement from your CME funds.
- Send all reimbursement receipts to Charlotte Baldwin (Accounting Technician) .
- For questions regarding CME funds, please contact Charlotte directly. The CME Policy is available in the ' T & E card information ' folder on SharePoint .
Steps to Request a T&E Card¶
- Complete T&E Training in CarolinaTalent My Learning
- o Part I: Computer-based training (best accessed in Google Chrome or Firefox; do not use Microsoft Edge).
- o Part II: Quiz requiring a score of 85% or higher to pass.
- Fill Out the Forms
- o Complete the first page of the T&E Application Form .
- o Sign the Agreement Form on page two.
- o Both forms are located in the SharePoint folder .
- Submit Forms
- o Email the completed application and signed agreement form to Angela Johnson .
- o Angela will forward your forms to the Travel Office.
- Next Steps
- o Once your forms are reviewed, you'll receive an email from the Travel Office with instructions on how to obtain and activate your new card. Please let us know once the card is available for pickup so we can arrange for someone to grab it for you.
